The Pediatric Center has received Patient-Centered Medical Home Certification

The Pediatric Center is excited to announce its recognition as a Patient-Centered Medical Home.   Following an exhaustive two year process, the National Committee of Quality Assurance (NCQA) awarded our practice with the highest level certification achievable.  We are the only pediatric practice in the greater Boulder area to have attained this recognition.

A NCQA Patient-Centered Medical Home is a model for primary care that combines teamwork and information technology to improve care and reduce costs.  Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits.  Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system.  Research has shown that the medical home model can lead to higher quality care at a lower cost, and can improve patient and provider reported experiences of care.

To attain the highest level of certification, The Pediatric Center demonstrated exceptional proficiency and competence in the following areas:

  • Care Management and Support
  • Care Coordination and Care Transitions
  • Performance Measurement and Quality Improvement
